• I voluntarily consent to receive services including massage therapy, skincare treatments, body treatments, waxing, and/or post-operative care.
• I understand that results vary and are not guaranteed.

• I understand that The Mindful Touch Massage & Wellness Suite and its providers are not medical professionals.
• Services provided are not intended to diagnose, treat, cure, or prevent any medical condition.
• I confirm that I have disclosed all relevant medical conditions, medications, and recent procedures.

      Treatment Acknowledgment

• I understand that some treatments may cause temporary side effects including, but not limited to:
• Redness
• Sensitivity
• Breakouts
• Peeling or flaking
• I understand that skin responses vary based on Fitzpatrick level, skin type, and treatment history.

             Client Responsibility

• I agree to follow all before and aftercare instructions provided.
• I understand that failure to follow instructions may impact my results.
• I accept full responsibility for my outcomes.
